How Hearing Works
Hearing is a remarkably elegant process, and understanding the basics can make sense of why certain types of hearing loss happen the way they do.
Sound starts as vibrations in the air. Your outer ear (the visible part, plus the ear canal) collects these vibrations and funnels them toward the eardrum, causing it to vibrate. Those vibrations pass into the middle ear, where three tiny bones — the ossicles, the smallest bones in the human body — amplify the signal and pass it along to the inner ear.
This is where things get really clever. The inner ear houses the cochlea, a fluid-filled, snail-shaped structure lined with thousands of microscopic hair cells. As sound vibrations move through the cochlear fluid, these hair cells bend and convert that mechanical movement into electrical signals. Different regions of the cochlea respond to different pitches, which is part of why we can distinguish a doorbell from a dog barking.
From there, the auditory nerve carries these electrical signals to the brain, where the real work of "hearing" actually happens. The brain interprets timing, pitch, and loudness, filters out background noise, and assembles meaning — turning electrical impulses into a voice you recognise or music you enjoy. Incredible!
It's worth noting that hearing loss can occur at any point along this pathway. Damage to the outer or middle ear (earwax build-up, fluid, structural issues) typically causes conductive hearing loss, which is often treatable. Damage to the hair cells in the cochlea or the auditory nerve itself causes sensorineural hearing loss, the most common type, often related to ageing or noise exposure, and usually permanent — though very manageable with the right support.
There's also growing research interest in how the auditory system and brain health are connected, which is why hearing care increasingly looks beyond just the ears.
Understanding where in this pathway a hearing difficulty originates is the first step toward addressing it effectively, and it's exactly what a comprehensive hearing assessment is designed to identify.
What Is Hearing Loss?
Hearing loss occurs when one or more parts of the hearing system are unable to efficiently detect or transmit sound to the brain. Although we often think of hearing as something our ears do, hearing is actually a sophisticated process involving the ears, the hearing nerve, and the brain working together as one beautiful system.
Sound begins as vibrations travelling through the air. These vibrations are collected by the outer ear and directed down the ear canal to the eardrum. The vibrations are then amplified by three tiny bones within the middle ear before reaching the cochlea—a remarkable, fluid-filled organ in the inner ear containing thousands of delicate sensory hair cells.
As these hair cells move, they convert sound vibrations into electrical signals that travel along the auditory nerve to the brain. It is the brain that ultimately interprets these signals, allowing us to recognise speech, enjoy music, appreciate environmental sounds, and determine where sounds are coming from.
Hearing loss develops when this pathway is disrupted. The problem may involve the outer or middle ear preventing sound from reaching the cochlea efficiently, damage to the sensory hair cells within the cochlea, changes affecting the hearing nerve, or, less commonly, the brain's ability to process auditory information.
The impact of hearing loss varies considerably. Some people notice only mild difficulty understanding conversation in noisy environments, while others struggle to hear even in quiet settings. Hearing loss may affect one ear or both ears and can develop gradually over many years or occur more suddenly.
Regardless of the cause, hearing loss is more than simply reduced volume. It often affects speech clarity, increases listening effort, contributes to fatigue, and can influence communication, social participation, mood and overall quality of life. Understanding how hearing works provides the foundation for identifying the cause of hearing difficulties and selecting the most appropriate management or treatment options.
Signs of Hearing Loss

Many people adapt to subtle changes over time without realising how much extra effort they are using to follow conversations. In many cases, it is a family member or friend who notices the changes before the person experiencing them.

You hear that someone is talking but struggle to distinguish individual words, especially in restaurants, cafés, meetings, or family gatherings. Voices begin to sound less clear, and people seem to mumble more than they once did.
Other common signs of hearing loss include asking others to repeat themselves frequently, increasing the volume on the television or radio, finding telephone conversations more difficult, or feeling tired after social interactions because listening requires greater concentration. Some people also become less confident in group conversations or start to think twice about attending a social situation or begin avoiding noisy social situations altogether.

Types of Hearing Loss
Not all hearing loss is the same. Understanding the type of hearing loss is an important part of determining its cause, whether it may be medically treated, and which management options are likely to provide the greatest benefit.
The three main types of hearing loss are conductive hearing loss, sensorineural hearing loss, and mixed hearing loss.
Conductive hearing loss occurs when sound cannot travel properly through the outer or middle ear to reach the inner ear. Common causes include earwax blockage, middle ear infections, fluid behind the eardrum, a perforated eardrum, or abnormalities affecting the tiny hearing bones. Depending on the cause, conductive hearing loss may be temporary or medically treatable. You may have heard of ‘grommets’ - these are common procedures conducted by an ENT specialist to ventilate and treat a middle ear diagnosis, for example.
Sensorineural hearing loss is the most common type, particularly in adults. It results from damage to the delicate sensory hair cells within the cochlea or, less commonly, the hearing nerve. Age-related hearing loss, long-term noise exposure, certain medications, and some medical conditions are common contributing factors. While sensorineural hearing loss cannot usually be reversed, many people achieve excellent communication outcomes through appropriately prescribed hearing technology and hearing rehabilitation.
Mixed hearing loss is a combination of both conductive and sensorineural hearing loss. In these cases, management may involve medical treatment to address the conductive component alongside audiological rehabilitation for the underlying sensorineural hearing loss.
A comprehensive hearing assessment allows your audiologist to identify the type and degree of hearing loss, helping determine whether medical referral, hearing rehabilitation, or a combination of both is the most appropriate next step.
Causes of Hearing Loss
Hearing loss can occur for many different reasons, and in many cases it is the result of a combination of factors rather than a single cause. Understanding why hearing loss develops is an important part of determining whether it may be medically treatable and how best to manage it long-term.
One of the most common causes is age-related change. As we get older, the delicate sensory hair cells within the inner ear naturally become less efficient. This gradual decline can affect clarity of hearing, particularly in background noise, even when overall volume still seems adequate.
Another major contributing factor is noise exposure. Prolonged or repeated exposure to loud sounds—such as industrial noise, machinery, music, or recreational activities—can permanently damage the hair cells in the cochlea. This type of damage often develops slowly and may not be noticed until communication difficulties become more apparent.
Hearing loss can also result from medical conditions affecting the outer or middle ear, such as earwax blockage, middle ear infections, fluid buildup, or perforations of the eardrum. In many of these cases, hearing may improve once the underlying issue is treated.
Other causes include certain medications that affect hearing, genetic factors, sudden changes in blood supply to the inner ear, and less commonly, neurological conditions affecting how sound is processed.
In many people, hearing loss is multifactorial, meaning several of these influences may contribute over time. Identifying the likely cause is an important step in determining the most appropriate treatment or rehabilitation approach.
Can Hearing Loss Be Reversed?
Whether hearing loss can be reversed depends entirely on the underlying cause and which part of the hearing system is affected.
Some forms of hearing loss can be partially or fully reversible, particularly when they are related to the outer or middle ear. For example, hearing loss caused by earwax blockage, middle ear infections, or fluid behind the eardrum often improves once the underlying condition is treated. In these cases, medical intervention can restore hearing back to previous levels.
However, the most common form of permanent hearing loss in adults is sensorineural hearing loss, which involves damage to the sensory hair cells within the inner ear. At present, these cells cannot be regenerated in humans. As a result, this type of hearing loss is generally considered permanent.
While it cannot usually be reversed, sensorineural hearing loss can often be effectively managed. Many people achieve significant improvement in communication through appropriately fitted hearing aids and structured hearing rehabilitation, which can help the brain make better use of the sound information it receives.
There is ongoing research into biological and regenerative treatments for hearing loss, but these are not yet available in routine clinical practice.
For most people, the key focus is not only whether hearing loss can be reversed, but whether it can be managed in a way that meaningfully improves communication, listening comfort, and quality of life.
How Hearing is Tested
A comprehensive hearing assessment is the most accurate way to determine whether hearing loss is present, identify its type and severity, and understand how it may be affecting your everyday communication. While many people notice changes in their hearing gradually, it is not always possible to determine the cause or extent of hearing loss without a detailed clinical assessment.
Your appointment typically begins with a discussion about your hearing concerns, medical history, lifestyle, and the situations where hearing is becoming more difficult. Understanding how hearing loss affects your daily life is just as important as measuring what you can hear during a test.
We will also examine your ears using an otoscope (and can often show you with a video-otoscope) to check for issues such as excessive earwax, infection, or abnormalities of the ear canal or eardrum that may require medical attention.
The hearing assessment itself includes immittance testing such as tympanometry, which is a test for eardrum function, pure tone audiometry, which measures the quietest sounds you can hear across a range of frequencies, and speech testing, which evaluates how well you understand spoken words. At Deane Audiology, we also believe it is important to assess hearing in conditions that more closely reflect everyday life. This includes speech-in-noise testing, such as the QuickSIN® assessment, which helps quantify the level of difficulty experienced in noisy listening environments. As we are also seeing more and more people presenting with tinnitus as their primary concern, we are also able to add extended high frequency (EHF) testing to a diagnostic assessment. EHF is uncommon in clinical practice, usually reserved for hospital clinics only but we have this capability and find it very important for demystifying tinnitus and speech processing issues, particularly for those who have trouble hearing speech in the presence on background noise. If indicated, more immittance testing is completed and clinical questionnaires may be administered.
Together, these assessments provide a detailed understanding of your hearing and communication abilities. Rather than relying on a single test result, we combine the findings with your lifestyle, communication goals, and listening requirements to develop recommendations tailored specifically to your needs.
Hearing Loss Treatment
The most appropriate treatment for hearing loss depends on its cause, type, severity and impact on lifestyle. While some forms of hearing loss require medical treatment, others are best managed through audiological rehabilitation and hearing technology. The first step is always an accurate diagnosis.
If hearing loss is caused by a medical condition affecting the outer or middle ear, treatment may involve earwax removal, medication, or referral to your GP or an ear, nose and throat (ENT) specialist. Addressing the underlying condition may restore or significantly improve hearing in these situations without the need to see an Audiologist ongoing.
For permanent sensorineural hearing loss, treatment focuses on improving communication rather than curing the underlying damage. Modern hearing aids are sophisticated devices designed to support the brain's ability to interpret sound more effectively, helping many people hear conversations more comfortably while reducing listening effort in everyday environments.
Successful hearing rehabilitation, however, is about far more than selecting a hearing aid. Long-term outcomes are influenced by a comprehensive assessment, careful technology selection, evidence-based fitting and verification, education, realistic expectations, goal-setting and measurement and ongoing professional support. The goal is not simply to make sounds louder, but to provide an appropriately tailored sound via the auditory nerve to stimulate the auditory cortex in a healthy way - improving the brain ability to process speech and sound naturally.
No two people experience hearing loss in exactly the same way. For this reason, treatment should never be based solely on an audiogram or the latest technology. The most successful outcomes are achieved when recommendations are guided by clinical evidence, objective testing, and a thorough understanding of how hearing loss affects your daily life.
With the right assessment and personalised care, many people experience meaningful improvements in communication, confidence, and quality of life, allowing them to participate more fully in the conversations and activities that matter most.
Early Action Matters
Landmark research published in The Lancet in 2011 (followed by others in 2017, 2020, and2024 ) marked the first time audiology was included in a major, multi-disciplinary ageing study in the United States. Over 12 years, more than 600 adults in their early 60s — all dementia-free at the outset — were followed into their 70s. While hearing was just one of many health factors measured, the findings were striking: untreated mild hearing loss was associated with a two-fold increased risk of developing dementia, moderate loss with a three-fold risk, and severe loss with a five-fold risk.
The study drew international attention, including the cover of The New York Times, and has since been reinforced by leading institutions such as Harvard Medical School.
